Service Agreement

Contract agreement for Equipment Repair Services between the following equipment owner, entered into the Client Name fields below (hereafter referred to as "Client") and Direct Target Products, Inc. (hereafter referred to as "DTP, Inc.") is made and entered into upon the date entered into the field below:

DTP, Inc. shall make sure that all repair services provided under the terms of this agreement are to be performed by properly trained individuals in accordance with the manufacturer’s recommendations and requirements.

In payment for services provided by DTP, Inc. the Client agrees to the following:

1. DTP, Inc.’s Rates as follows: $95.00/hour including travel time to and from Client’s facility. In addition to hourly charges, materials (grease, oil, etc.) and machine parts will also be charged. The aforementioned rates billed in quarter hour increments. Minimum charge for a service call is $95.00/Hr travel and 1 Hour labor at $95.00/Hr.
2. An hourly rate of $140.00/hr will be charged for repair calls that fall outside of normal operating hours of 7:00 AM-5:00 PM, Monday-Friday EST.
3. The Client is of the understanding that parts and supplies along with other materials should be of exact specifications described by DTP, Inc. and/or the manufacturer for the equipment to reach optimal operating status. The Client shall also purchase from DTP, Inc. all required parts, supplies and other materials that may be necessary and reasonable in order to properly maintain the equipment in proper working order.
4. Payment terms are as follows: Clients that have an open account with DTP, Inc. will be 1% 10, net 30 days. DTP, Inc. also accepts Visa, Amex, MasterCard, and Discover Clients without an open account will be required to fill out a credit application. If you would like to have an open account with DTP, Inc. we request that a credit application be filled out. Credit approval will be turned around as timely as your creditors provide information.
