Net 30 Account Request

As part of your application for a Net 30 Term Account with Direct Target Products, Inc., we request your authorization to potentially pull your business credit report. You understand that by applying for a Net 30 Term Account and clicking on the appropriate disclosure, you are providing "written instructions" to Direct Target Products, Inc. under the Fair Credit Reporting Act authorizing Direct Target Products, Inc. to obtain information from your personal and/or business credit profile or other information from one or more consumer reporting agencies.

You authorize Direct Target Products, Inc. to verify information in your application, and you agree that Direct Target Products, Inc. may contact third parties to verify any such information. Direct Target Products, Inc. may use your consumer reports to authenticate your identity, to make credit decisions, and for related purposes. You authorize Direct Target Products, Inc. to obtain consumer reports each time you request a Net 30 Term Account.

You further acknowledge that Direct Target Products, Inc. may report credit data on your account to credit reporting agencies. Direct Target Products, Inc. agrees that any information furnished will be accurate. You understand that, if you are applying for a Net 30 Term Account the credit pull and reporting on your Net 30 Term Account may affect your credit score with Experian, Equifax, or Transunion or Duns & Bradstreet.
